Easy steps for your consultation

Prepare for Your Office Visit

Please arrive 30 to 45 minutes prior to your appointment to complete your intake forms and all necessarily forms requiring by your insurances and federal/state requirements.

Please remember to bring the all CDs of your images, all pertinent medical records, test results and a list of current medications and dosages to your first consultation.

Many new patients find it helpful to bring a list of questions as well. Further, we also welcome you to bring a family member or friend to your appointment.

Finally, please be sure to bring the following items to each visit:

  • A list of current medications and dosages
  • All CDs of your radiographic images MRIs, XRs, CTs along with the reports
  • Insurance card(s)
  • Co-pay (We accept Cash, Discover, Visa and MasterCard)

Cancellation Policy

In consideration of other patients who are in need of an appointment and to ensure that our practice operates smoothly, we request that you call the office at least 24 hours in advance to cancel an appointment. While we understand that circumstances may arise, we do reserve the right to bill for scheduled time when appointments are not cancelled as requested. We appreciate your understanding and cooperation.
